I'd recommend against doing this survey unless revised

I understand the purpose but it is rigged, and so full of errors! The target group is unclear ("for posterity" is supposed to mean it is really for SG's only) The control/test questions are too naively formulated. With many vital questions, there are not enough options and you are forced to choose yes or no to things when maybe neither answer is applicable. There is no opting out of any question or you will not be able to continue to the next page. Sometimes two or more statements of fact are combined into a single question which you have to answer yes or no to, when you might need a yes for one and n/a for the other. The spelling is poor. The formula unsound and wording is fixed for arriving at conclusions the you may not want to contribute to. The survey hasn't been carefully thought through. There is no order of heirarchy so that answering in negative to one question will rule out being asked about a sub-detail of the same topic later, and then being forced to answer yes or no to something you shouldn't need to be asked. There is no recourse for contacting the people behind the survey, and no telling how they use your data or what information they keep about you.
