I visitedf your site and I can see your good intentions, nevertheless your questions are wrong. In order to get meaningful "overall view" you need to have questions that are orthogonal. This means that they are exclusive to each other. In other words that they don't overlap.

Here is a suggestion: Ask people about those questions and work on them before going to collect your data. This is called validation by survey professionals and without it, whatever data you get will not reflect much good of anything other than your own predetermined views.

For example: Did you leave because:
1: You were unhappy
2. Life was hard

One of those will lead to the other so the answers will not really reflect reasons.
