From what you report, it is possible Kelly has a mood disorder with delusional features. Some of what you describe is also characteristic of a dissociative disorder, i.e., speaking with Grandpa's spirit before breakfast and after coitus.

What are his drinking and drug habits? This guy sounds like a good candidate for alcoholism/addiction.

If I had the time and inclination, I could do some psychodynamic speculation about Berg as a father figure and masculine identity conflicts. However interesting that form of analysis may be, I don't believe it's helpful for understanding brain disease. Psychodynamic theory is more useful for understanding personality/character issues. Even there, I'm not sure I completely buy into the psychodynamic approach as the most expedient way of understanding character pathology.
