Crystal might be her "Bible name" (although I don't recalll the name "Crystal" actually appearing in the Bible) rather than her legal name.
KG may stand for Katherine Gambrill. If her name now is Katherine Ferrell and she was previously married to a Michael Gambrill then her surname may have been Gambrill at the time she submitted her affidavit to the High Court.
